Registration Procedure

The following paperwork must be completed and signed by the parent(s) before the admission of a child to the Midland Childcare Centre:

1. Completed application form
2. The child and parent’s information
3. Immunization form, a copy of your child’s health card (optional), and medical consent form
4. Consent forms: medical response, neighbourhood walks, and photo/video
5. Signed acknowledgment in the application form that you have read and agree to our terms of service, as outlined in the parent handbook.

In addition, a parent interview shall also be conducted by the Supervisor with one or both parents.

Please note that a 2 week deposit and application fee $50 is required at the time of the interview. Keep in mind that registration fee and deposit are non-refundable.
